I am a newly qualified teacher, currently teaching English in a secondary school here in Dublin. First I like to get a feel for where they want to improve, what they are studying specifically in school (books, poetry etc.). Once I finish that, I go through what the plan for improvement is for that student. When teaching I go through that area step by step and ensure that the student truly understands before moving on.
